GBB FORMS

This Page has links to downloadable forms provided and approved by GBB National, for use by GBB Units.  Forms will be added to this page as they are updated and approved.  Before using a form, always check this page for updates.

National Event Registration Form (updated March 30, 2008).  This form is in Rich Text Format (.rtf).  It is to be printed an submitted for National Events such as Rendezvous and High Adventure. Note -- 9MB, allow sufficient time for download.

Medical Permission Form (updated April 9, 2008).  The medical form should be completed each year and submitted with the annual registration. Each GBB unit should have original current medical forms on file for their registered men and boys. A copy should be sent to district with annual registration. District should forward a copy of the form to National Headquarters with annual registration.  It is in Rich Text Format (.rtf).  Here is the same document in MS Word Format.

Event Permission Slip (updated October 20, 2007).  This form is in MS Word format (.doc).  It is to be printed and included with the Medical Permission Form.

Annual Registration Forms (updated October 20, 2007).  This is the form, in MS Word format (.doc), that is to be used by each unit to report the details of their membership to GBB National each year

Volunteer Staff Application (updated March 30, 2008).  Rich Text Format (.rtf).  This application must be reviewed and completed by each current leader, immediately.  The renewal (addendum) will only need to be completed in subsequent years.  Initially, however, the National Board has determined that ALL PEOPLE who wish to work with boys under GBB will need to complete this entire form.  3MB download.

Youth Protection Policy (uploaded April 9, 2008).  Rich Text Format (.rtf).  This policy is referred to in the Volunteer Staff Application, and each applicant is responsible for reviewing it.  Here is the same document in MS Word format.

Expense Report (updated April 9, 2008).  Rich Text (.rtf) format.  This expense report form is used by GBB National, and may be adapted by the Districts for their purposes.  Here is the same document in MS Word format.

Request for Materials (uploaded March 30, 2008).  This form is in Rich Text Format (.rtf).  This form is to be completed and mailed to National Headquarters or e-mailed to Director of Boys Ministry Rob Gable.  Note -- 5MB, allow sufficient time for download.
